Issuance of certificate
How to apply
For those who have graduated from, withdrawn from, or are currently on leave of absence from Future University Hakodate and require a certificate,Certificate Issuance Request FormPlease fill in the required information, enclose the certificate fee, a self-addressed, stamped envelope, and a copy of your identification, and submit your application by mail.
Matters
- Name (phonetic)
- Date of Birth
- Address (postal code, phone number)
- Name of graduating department, student ID number, graduation (completion) date.
- Required certificates and number of copies
*For other certificates (other than academic transcripts and graduation/completion certificates), please contact the Admission/Public Relations/Career Support Division in advance. If the requesting organization has a specific form, please specify how to fill it out and include that form with your request. - Where to submit and purpose of use
Certification fee
300 yen per copy.
Payment will be made by postal money order issued by the post office. Please do not write anything on the postal money order. Please be sure to send a postal money order for the exact amount of the fee.
Reply envelope
A standard size 3 envelope with the mailing address written on it and postage stamps affixed. (Regarding postage, it's 110 yen for up to 4 copies; for 5 or more copies, a non-standard envelope will be used, and please inquire about the postage.)
Copy of identification
Please enclose a copy of a document that can verify your identity, such as a driver's license, health insurance card, or passport.
Number of days required for issuance
After confirming your application, it will take approximately 3 business days to send you your academic transcript and graduation/completion certificate, and approximately 1 week for other certificates. Since the process will be conducted by mail, please apply well in advance.
